Outreach

Marana Health Center has created a department specifically dedicated to guiding patients in need of financial assistance.  Our Outreach Coordinators are available by appointment to personally guide patients through the qualification of both government programs and our discount program (“Sliding Fee Scale”).  And, when you are nearing the termination date of any of these programs, our staff will happily work with you to renew the program and explore what is available to you and your family! 

Scheduling your appointment 

It is recommended you complete your Outreach appointment prior to your medical appointment to allow for optimal coverage, and ensure your visit will be covered by a financial assistance program, if you qualify.  Please note some programs can be back dated, but others cannot.  You will be requested to complete and sign a Self Pay Agreement form prior to your medical appointment pending approval of the available programs.  Concessions can be made based on medical need, but please speak to our staff about options prior to your appointment.  We are here to help!

Please call our scheduling department and provide some basic information, allowing us to coordinate your appointment with our Eligibility Coordinators.

  • How many people are in the household?
  • Is this a new screening or a renewal?
  • Would you like assistance with Medical, Dental, Food Assistance, or all three?

Preparing for your appointment 

Please have some of the following items ready for your Outreach appointment to help expedite the process.  This is of particular importance in the application process of all programs, and will oftentimes determine coverage.  We are able to assist patients without some of the listed items, so please do not let the lack of documentation deter you from attending your appointment.  Our coordinators may be limited on the programs available without the supporting documentation, but will work diligently with you.  The more the better!

  • Social Security Cards
  • Numbers for all applicants or proof of applications for a SSN
  • Photo Identification
  • Driver’s License
  • Photo ID issued by federal, state, or local government,
  • Tribal ID
  • School ID with picture
  • Citizenship
  • US Birth Certificate or Certified Copy
  • US Naturalization Certificate
  • US Passport
  • Residency/Residential Address
  • Utility bills with name and address showing residency
  • Income
  • ALL money received by household from any source last month and this month
