Our commitment to clinical excellence is rooted in a goal of providing comprehensive primary care that elevates the health status of patients, their families and communities. We care for patients across the lifespan, from pre-conception to Geriatric care, with an integrated model of three core services - medical, dental and behavioral health care. These core services are supported by additional services, including nutrition counseling, tobacco cessation management, breastfeeding support, telephonic disease management, diabetes education, comprehensive school-based services, clinical pharmacy, and case management. Using a health information technology framework and electronic health record/practice management system, we deliver culturally competent, linguistically appropriate:

• Interdisciplinary care (medical, dental and behavioral health care) so patients can receive immediate, seamless and coordinated service without redundancy or error;
• Timely and accessible care with extended hours to improve the patient experience;
• Planned medical care with team-based care delivery and coordination, focusing on chronic disease management;
• Comprehensive primary oral health care with a focus on a full range of Pediatric, Adult and Geriatric patient services, care coordination;
• Patient-centered behavioral health care with the option of receiving individualized, family or group therapy; care coordination and special programs for children

The core of MHC’s health care is comprehensive primary care for individuals of all ages and at all stages of life. Family medicine is the core, with internal medicine, pediatrics, women’s health, pharmaceutical and ancillary services (radiology and laboratory) available to all our patients.  Additional supports such as nutrition counseling, breastfeeding support, diabetes education, and case management support the core services. Recognizing the importance of behavioral health care in primary care, MHC has an integrated model of behavioral health and primary care with psychiatric specialists for both adults and children practicing along the medical team. The interdisciplinary team is an essential element of our planned care model.
